Your family doctor works together with a team of specialists, a nurse and a dietitian, ensuring that your entire treatment process is carefully planned and supervised.

Who is coordinated care for?
The program is available for patients with:
- cardiological conditions (hypertension, heart failure, coronary artery disease, atrial fibrillation),
- diabetes and prediabetes,
- endocrine disorders (hypothyroidism, hyperthyroidism, single or multiple thyroid nodules),
- pulmonary diseases (COPD, asthma),
- chronic kidney disease.
